Public health agencies are required to complete a community health assessment (CHA) and community health improvement plan (CHIP) every five years. This process of assessment and planning assures that public health departments are being responsive to changing community needs and allows them to assist in effectively determining evidence-based actions to address health outcome gaps.
The CHA is part of an ongoing broader community health improvement process. A community health improvement process uses CHA data to identify priority issues, develop and implement strategies for action, and establish accountability to ensure measureable health improvement, which are often outlined in the form of a CHIP.
Community organizations, businesses, schools, health systems, and many other cross-sector partners come together to collaboratively address health issues and implement the strategies highlighted in the CHIP.
Oneida County Health Department is currently working under the 2017 CHIP which has three health focus areas: mental health, alcohol and other drug abuse, and chronic disease. Additionally, the plan has overarching focus areas that include health equity, access to care, and health across the lifespan.
